- Masshealth:
All recipients need to be screened and approved in order to attend
adult day health. Masshealth recipients must agree to attend at
least two days per week, 6 hours per day.
a) If a recipient receives home services by a certified
home health agency, their registered nurse completes a long
term care assessment form and sends it to the Aging Services
Access Point (ASAP) for screening approval.
b) If a recipient does not receive home health agency services,
then an on-site assessment for screening is done by the ASAP registered
nurse. See phone numbers listed below.

Colony Elderly Services Contract
a) Old Colony Elderly Services will
pay for Adult Day Health Care for one of their clients, who is
deemed appropriate by the case manager and/or the registered nurse
at Old Colony Elderly Services.
b) When Old Colony Elderly Services pays for their client
to attend, no screening is required.
